Cancellation Policy
No-Show / Cancellation / Refund / Return Policy:

* There are no refunds or returns on appointments, products or gift certificates purchased.
* All massage treatments are by appointment only. Because your massage session is reserved exclusively for you, BE WELL Massage Therapy, LLC requires 24-hours advance notice when canceling or rescheduling your appointment.
* If you do not provide a 24-hour notice to cancel or if you do not show up for your appointment, you will be charged 50% of your missed appointment.
* No-shows will be required to leave a credit card on file or pre-pay before booking future appointments.
* This policy will be strictly enforced to ensure our time together is valued, honored and respected. Please plan appropriately. Thank you!
Appointment Policy
* All massage treatments are by appointment only.
* Please note that if you are late for your appointment, you will be required to pay for the full appointment. I will do everything I can to make sure you receive the full service but please be aware that your time may be shortened if there are other appointments scheduled after you.

Payment for Services
* BE WELL Massage Therapy, LLC does not participate in any insurance plans and cannot accept any form of insurance as payment for services.
* It is the sole responsibility of the client, or parent/guardian in the case of minors, to render payment in full at the time services are received. Payment may be made by cash, check, credit card, Paypal or Venmo. There will be a $35 fee for returned checks.
Massage Treaments
* Massage practitioners are not qualified to perform medical evaluations and cannot diagnose or treat any physical or mental illness. Therefore, nothing said in the course of the massage session should be construed as such. Please see a physician, chiropractor or other medical specialist for any physical or mental ailment of which you are aware.
* Because massage therapy should not be performed under certain medical conditions, it is important that every client complete the client intake form and answer all questions completely and honestly. Also, please keep your massage practitioner updated as to any changes in your medical profile.
* Any illicit or sexually suggestive remarks or advances made by any client will result in the immediate termination of the massage session, and client will be liable for payment of all scheduled services.
Confidentiality
* The privacy of your personal information is important to my practice. I am committed to collecting, using and disclosing personal information responsibly and only to the extent necessary for the services I provide. Information regarding your treatment is confidential and will not be released without your written consent, unless required by law. All information pertaining to minors will be released to their parent or legal guardians upon their request.
